I find it easier to change file names directly instead of right clicking and choosing RENAME. I change the file name by single clicking the file name, waiting a second so it doesn't open, then click in the name field and update the name.
Today I wanted to update the version name of a file from "*** ver0.pdf" to "*** ver1.pdf".
When I did this, the screen updated to the new Ver1 name. Great!!! but no so fast.
When I reopened the file, the file name that appears at the top of the Adobe Reader window still said Ver 0. That's not right.
I went back and looked at the XYP window and it said the name of the file was Ver1.pdf. So then I pressed F5. The XYP list didn't change and when I reopened the file the old file name was still in the Adobe Reader header. Then I went up one directory and back down. The XYP list still said it was called Ver1 but when I opened the Adobe frame header still said Ver0. Next I completely closed XYP and reopened it. The file list still said the file name was Ver1 but when I reopened Adobe and the header still called it Ver0.
At each of these attempts while looking at the file in Adobe, I looked at the file properties and the under Description>Title: the old Ver0 name was listed. When I changed the description Title to Ver1, closed the file and reopened it, now the header was correct. Looking at other pdf file properties the Title field is blank. The only time the Title field had text was on files I had changed the name. In all other cases for all of my PDFs the Title field was blank.
Ok now if that wasn't weird enough how about this:
After changing the file name to Ver1 in XYP I checked the file name using Windows File Explorer (WFE) and the file name there was correct i.e. Ver1 BUT when I opened the file, the Adobe header was still Ver0. Changing the file name in WFE to Ver2 changed the name in both WFE and XYP but Adobe still said Ver0. Finally, going back to WFE I changed a different PDF and it was correctly sorted alphabetically and it opened in Adobe with the correct name.
I'm not a systems guy or coder but it appears that XYP is not passing the name change to Windows correctly, or if it is, the name it not updating properly. This brings up another issue that bugs my but when using Windows File Explorer, if you change the name, the window automatically refreshes. The new file name is displayed and it is sorted to the proper place in the list of files i.e. if you changed the name from "Jan 2 2023" to "Jan 2 2024" it would place Jan 2 2024 in the correct alphabetical location.
I don't know where the problem is but because name changes made in WFE work and don't when using XYP I'm pointing the finger at XYP. There is a name for what my wife does which is she automates software for taxes. They write code to scrape a PDF to find the number next to Amount Due. Then they open an Excel spreadsheet and paste the amount due along with other data, then they do this 100 times and finally they shift the information to the actual tax software to calculate the true tax and finally it is output in a way the the government electronic tax software can read it. Any changes or updates to any of the various software packages can completely screw this up. I bring this up because I mentioned 3 different pieces of software but still think the root cause is in XYP
At the end of the day, If I can't have 1000% confidence that the name was changed and propagated through out the entire system I need to stop using this feature which is kinda important.

Firstly, you really should try to make your posts shorter or you won't get too many responses (apart from TLDR).
PDF's (like other files as well) have properties including a file name that is usually written on the initial save of the file.
Changing the filename in the filesystem does not update all file properties and I assume that Acrobat like other PDF readers uses the (unchanged) name from the properties to display in its title bar.
